Continuity of Care: Your patients remain under your care while receiving specialized treatments
at Allevate Health. We keep you informed throughout their journey, ensuring seamless communication and optimal
outcomes.

How to Refer: Fax referrals to 732-360-6579. For SPRAVATO® treatment insurance coverage, please
include:

A recommendation for SPRAVATO® treatment.
Confirmation that the patient has treatment-resistant depression.
A list of at least two recent antidepressant medications the patient has tried without success.
Your NPI number.
